Matthew John Mulyk, age 42, passed away with his family by his side on Sunday, October 4, 2026.

Beloved husband of Mia D. Perkins;  Adoring father of Dakota & Mila Mulyk and Loving step-father of Kayla Chasing Horse.   Cherished son of Michael J. and Deborah A. Mulyk.  Dearest brother of Mark (Andrea) Mulyk and Nicole (Josh) Skelly.  Favorite uncle of Nicholas & Sofiya Mulyk and Roman Skelly.  Fond son-in-law of Robert & Agatha Lefler.  Dear nephew and cousin of many too numerous to mention.   Matthew will be greatly missed by his many friends, neighbors and colleagues.     Memory Eternal!

Matthew was a dedicated Firefighter / Paramedic with the Chicago Fire Department most recently serving with the men and women of Truck 55 / Engine 119 / Ambulance 39 .

A Memorial Visitation will be held on Sunday, October 18, 2026 between the hours of 2 - 6 P.M. at Muzyka Funeral Home, 2157 W. Chicago Ave. ( at N. Leavitt St. ), Chicago, 60622.

A Celebration Of Life will immediately follow the Memorial Visitation between the hours of 6 - 9 P.M. in Ukrainian Village at the American Ukrainian Youth Association  ( CYM ), 2457 W. Chicago Ave. ( at N. Campbell Ave. ), Chicago, 60622.
